Q) How do I submit my story?

​

Each time enough donations have been raised to provide a ScoliSMART Activity Suit to a scoliosis patient in need, we will announce it on our Facebook page (Crooked Life Foundation) with instructions on how to submit your story for consideration, so we suggest following that page.  In addition, we will add a "Submit Story" button on the Home page of this website to indicate when we are accepting submissions, so check back often.  One patient will be selected from the story submissions each time we raise enough funds to provide a suit to a patient in need.

Q) Do I have to submit my story each time a suit is being provided to a patient in need, or can I submit it just once?

​

Individual circumstances can change, so it is necessary to submit your story each time you are eligible and would like to be considered.  Again, one patient will be selected from among the story submissions each time we raise enough funds to provide a suit to a patient in need.

Q) What does the selected recipient of each ScoliSMART Activity Suit receive?

​

The recipient receives a fitting consult, x-ray (if needed), ScoliSMART Activity Suit, and a very warm welcome from The Crooked Life Foundation at the location of fitting (Value $995).  Note: Travel expenses to the fitting location are not covered.

Q) If selected, where would I need to travel to be fitted for the suit?

​

The Crooked Life Foundation is located in Missouri; therefore, most fittings take place at ScoliosisKC in Belton, Missouri.  We understand not everyone can travel to Missouri, so in an effort to allow more people to have the opportunity to submit their story we may offer other fitting locations from time to time.  When we offer other fitting locations that means that The Crooked Life Foundation directors are traveling at their own expense to meet the recipient on a particular date and time in another state, so no changes can be made to the fitting appointment (date or time).

Q) If I'm selected as a recipient, but have a different fitting center closer to me, may I get fitted at that location instead?

 

Please only submit your story if you're able to travel to the fitting location indicated on the application form (if you're selected as a recipient). The Crooked Life Foundation is present for each fitting to cover the costs of the fitting, greet the recipient, take photos, do a video interview, and get benefit paperwork signed.  The recipient must travel to the location indicated by The Crooked Life Foundation.

Q) How old does a scoliosis patient have to be in order to submit their story?

​

The patient must be 18 years old.  The suit is only a "stand alone" treatment option for adults.

Q) Can I submit my story if I live outside the United States?

​

We are a U.S. Nonprofit Organization providing ScoliSMART Activity Suits to patients unable to afford them.  Travel costs would be unaffordable (for qualified entrants) outside the United States; therefore, one must reside in the United States.

Q) What are the qualifications to submit your story?

​

  • Patient must be 18 years old.  The suit is a "stand alone" treatment for adults.

  • Must reside in the United States.

  • Must fill out the application form completely.  

  • Must have scoliosis, and be a good candidate for the ScoliSMART Activity Suit. *Upon selection, the recipient will need to provide an x-ray picture (less than 5 years old) to the fitting doctor to verify this.  

  • If selected - must be able to travel to the indicated fitting location by the date/time indicated - travel expenses not included.

  • Family income must be below 400% of the Federal Poverty Level (see below), and must describe in the application the financial barriers that prevent the purchase of a Suit. 

    • 1 person household with income below $48,560

    • 2 person household with income below $65,840

    • 3 person household with income below $83,120

    • 4 person household with income below $100,400

    • 5 person household with income below $117,680

    • 6 person household with income below $134,960

    • 7 person household with income below $152,240

    • 8 person household with income below $169,520
